a.  CSC Form 212 Revised 2017 in two copies with the latest 2×2 ID picture ( DOWNLOAD: CSC Form 212 Revised 2017 )

b. Certified photocopy of PRC professional identification card or a PRC certification showing the teacher’s name, LET rating, and other information recorded in the PRC Office

c. Certified photocopy of ratings obtained in the Licensure Examination for Teachers (LET)/Professional Board Examination for Teachers (PBET)

d. Certified copy of transcript of records

e. Copies of service records, performance rating, and school clearance for those with teaching experience. If unavailable, the applicant must submit a justification citing the reason/s for unavailability.

f. Certificates of specialized training, if any

g. Certified copy of the Voter’s ID and/or any proof of residency as deemed acceptable by the School Screening Committee

Tanong ko lang po sana until now I have not yet received my UAN. I registered yesterday (Monday) but I’m afraid I will still not receive it after 48 hours from registration. I hop you can assist me on this po. I am running a deadline, dito po sa Division of Cagayan de Oro City, the deadline is on April 10. Thank you po.

By the way, I read on the guideline released by DepEd last March 27, 2015 that school principals can still accept applications even if some papers or requirements are not yet completed, am I right po ba? Is it possible for me to submit the other requirements at the division office po ba I will just ask the principal to put a remark on my application. To follow na po yung ibang requirements. Pwede po ba yun? I will greatly appreciate your enlightenment on this. God bless. More power DepEd.

Sir sa mismong DepEd website napo yung may problema.. hanggat hindi nila inaayos yung system hindi po ito mareresolba. Pagdating naman sa username or password might be incorrect lumalabas po yan pag hindi pa po verified yung email address niyo.

When a student applies to college, he or she will need a letter of recommendation of at least two from a high school teacher, or sometimes an employer. College admissions offices look for certain things in a student's letters of recommendation.

How to write a letter of recommendation

In order to write a letter of recommendation in an approved and professional form, you must pay attention to:

  • The introduction includes a greeting to the donor.
  • Introducing yourself - a brief personal biography - in the next section.
  • Introducing oneself - academic, school and university biography -
  • Give the reason that prompted you to recommend this person to the relevant authority
  • The activities that your student performed during your teaching period
  • Why do you think this student deserves recommendation?
  • Finally, provide your contact information
  • Your name + signature.

Types of recommendation letter

  • First: A letter of recommendation from a secondary school teacher who has supervised the teaching of one subject, such as mathematics, for example, or more than one subject, for example, physics and chemistry.
  • Second: A letter of recommendation from a teaching assistant, doctor, or professor who taught you at the university or supervised your teaching in one year for one subject, for example, or more than one subject, for example, a teaching assistant for mathematics, anatomy, or even biochemistry, for example.
  • Third: A letter of recommendation from your employer if you work for a private or public institution.

The first step

The second step

The third step

In the next line, it will be written to you, for example, if you are diligent in class, have personal activities while studying, or even social activities with your classmates, for example:

While a student at ABC High School, Ahmad has always challenged himself academically. He is an active participant in class discussions and grasps the material quickly. He has superb written and verbal skills that are a pleasure for any teacher to encounter. Ahmad even tutors incoming high school freshmen who are struggling with their English courses.

The fourth step

If he wants to talk about your practical activities, which is a very good thing when writing a recommendation, he may use mentioning the objectives of the activity that you carried out alone or with classmates, for example:

Ahmad also excels in extracurricular activities. Ahmad has served as the features editor of our high school paper for the past two years and has written a number of insightful, thought-provoking articles. Her ability to delegate work to her assistant editors also reflects his organizational skills and strong leadership abilities.

Finally, he will conclude by saying that if you obtain this acceptance or scholarship for the purpose of the thesis, it will be a very impressive thing for the university and for you as a student alike. For example, he will write saying:

Ahmad would bring so much to your University, both inside and outside of the classroom.

The teacher or teaching assistant will write his address, phone number, and email, and if possible if they have a website, as in the following line:

Send a letter of recommendation for the scholarship or university

There are three methods: They depend on the method of application to the entity, whether a university or an institution:

You may be asked to send a letter of recommendation either by mail or by uploading it via electronic application in a file format PDF

You may be asked to send it along with the other required papers via regular mail, in paper form.

Third method:

When you submit an application to the university online, for example via the electronic application link, you are asked to write an email and the name of the person who will write you the recommendation letter. In this case, a message will arrive on his email asking him to click on the link that was sent to him via email to do so. He himself fills out or writes the recommendation letter electronically, and here there is no need to upload it in PDF files or send it as a paper form via mail.

A second example of a letter of recommendation

I am writing to wholeheartedly recommend [Student Name] for admission to your esteemed university. As [his/her] academic advisor for the past three years, I have had the pleasure of watching [him/her] grow and develop both academically and personally.

[Student Name] is a highly motivated and dedicated student, consistently earning top grades in all of [his/her] courses. [He/She] is a critical thinker and always willing to engage in class discussions, bringing unique and valuable perspectives to the table. [His/Her] natural curiosity and desire to learn has led [him/her] to consistently seek out additional resources and opportunities to deepen [his/her] understanding of course material.

In addition to [his/her] strong academic performance, [Student Name] is also highly involved in extracurricular activities. [He/She] has served as the president of the school's debate team for the past two years and has led the team to numerous victories at local and regional competitions. I-131F, Application for Parole in Place for Certain Noncitizen Spouses and Stepchildren of U.S. Citizens

ALERT: On Aug. 26, 2024, the United States District Court for the Eastern District of Texas, in Texas v. Department of Homeland Security,  Case Number 24-cv-306 administratively stayed DHS from granting parole in place under Keeping Families Together for 14 days.

To comply with the district court’s administrative stay, USCIS will:

  • Not grant any pending parole in place requests under Keeping Families Together.
  • Continue to accept filings of Form I-131F, Application for Parole in Place for Certain Noncitizen Spouses and Stepchildren of U.S. Citizens.
  • Continue to schedule biometric appointments and capture biometrics at Application Support Centers (ASCs).

The district court’s administrative stay order does not affect any applications that were approved before the administrative stay order was issued at 6:46 p.m. Eastern Time on Aug. 26, 2024.
